BOOST Home Health provides physician-directed home health services delivered by licensed and certified healthcare professionals. Each plan of care is individualized based on comprehensive assessment, physician orders, and patient-specific needs, and is provided in the comfort of the home.
Skilled Nursing
Care provided by Registered Nurses and Licensed Vocational Nurses, including medication management and teaching, wound and ostomy care, catheter care, IV therapy and infusion services, lab draws, coordination of mobile imaging, and pre- and post-operative care. Nursing visits focus on patient education, safety, and clinical monitoring to support recovery and independence.
Physical Therapy
Therapeutic services designed to improve strength, balance, mobility, and endurance. Physical therapy includes gait training, fall prevention strategies, and education on the safe use of durable medical equipment to enhance functional independence.
Occupational Therapy
Occupational therapy focuses on improving upper-body function and daily living skills. Services support patients in safely performing activities of daily living and adapting their home environment to promote independence.
Speech Therapy
Evaluation and treatment of communication, cognitive, and swallowing disorders related to medical conditions. Therapy is tailored to each patient’s abilities and goals to improve safety and quality of life.
Medical Social Work
Assessment and support of social, emotional, and psychosocial needs related to illness. Services include care coordination, counseling, and assistance with community and financial resources to support safe living arrangements.
Home Health Aide Services
Assistance with personal care activities such as bathing, grooming, dressing, ambulation, and meal preparation. Services are provided under a supervised plan of care to support comfort and daily functioning.
